Peru requires health affidavit form for travelers entering the country

People wishing to enter Peru shall fill out an online health affidavit and self-isolation commitment form, which can be found on the website of the National Superintendence of Migration, the entity reported on Tuesday.

The affidavit must be filled out within the 72 hours prior to travel and shared —under terms of confidentiality— with the Ministry of Foreign Affairs and the Ministry of Health, according to Ministerial Resolution No. 627-2020-Minsa.

The affidavit can be completed from any computer or mobile device connected to internet by simply visiting the Migration office's website.

The document will be requested —starting today, August 18— to passengers entering the country through the Jorge Chavez International Airport, as part of the resumption of flight operations at Peru's principal air terminal and the implementation of a protocol for travelers entering and leaving the country on special flights.

The above-mentioned regulation provides that the online filing of the affidavit is mandatory in order to enter the South American country. 

The National Superintendence of Migration has implemented immigration control on Peruvian citizens and foreigners who return or leave the country with the authorization of the Government, after Peru closed its borders to prevent the coronavirus (COVID-19) from further spreading across the country in the first half of March.

This control is carried out in coordination with the Interior Ministry (to which the Migration office is attached), the Ministry of Foreign Affairs, the Ministry of Foreign Trade and Tourism (Mincetur), the Ministry of Health and the National Civil Defense Institute (Indeci).
